Introduction Letter<\/strong><\/p>\n\n\n\n You have been invited to participate in a research survey to test the reliability of a research tool called (EXPLOR)-5 <\/p>\n\n\n\n This survey is important because little is known about the best approaches for engaging older adults in the adoption and maintenance of physical activity, and the information gathered through this survey will help fill that knowledge gap.<\/p>\n\n\n\n As a participant in this study, you will be asked to fill out a survey, approximately two weeks apart. Your participation in this study will not pose any harm to you. <\/p>\n\n\n\n Your participation in this study is entirely voluntary, and you may stop at any time, without consequence, and the information you provided will be removed and destroyed. <\/p>\n\n\n\n All information collected during this survey will remain confidential, and you cannot be identified from any of your responses. You will only be identified by a number in the analysis and the final transcript. You can stop at any time and there will not be any repercussions.<\/p>\n\n\n\n INTRODUCTION<\/strong><\/p>\n\n\n\n You are being asked to consider taking part in this study because research suggests that Canadian adults do not achieve recommended amounts of physical activity for overall healthy living. <\/em>This study will look at a new tool as a possible factor in understanding the facilitators and barriers of physical activity participation in the older adult population. <\/p>\n\n\n\n WHAT IS THE USUAL TREATMENT?<\/strong><\/p>\n\n\n\n Taking part in this study will not change the usual activities\/treatment.<\/p>\n\n\n\n WHY IS THIS STUDY BEING DONE?<\/strong><\/p>\n\n\n\n The purpose of this study is to understand the reliability of a proposed tool that may help us understand physical literacy of adults in community settings. It may help us understand how to help adults be more active and improve health outcomes.<\/p>\n\n\n\n WHAT WILL HAPPEN DURING THIS STUDY and WHAT ARE THE RESPONSIBILITIES OF STUDY PARTICIPANTS?<\/strong><\/p>\n\n\n\n If you decide to be in this study, you will be asked to complete a questionnaire twice, 2 weeks apart. The questionnaires will be completed at 2 time points and will be completed online.<\/p>\n\n\n\n A total of 25 adults are expected to participate in this study between February 2021 and April 2021. The total time commitment for each patient is approximately 30 minutes (15 minutes for each questionnaire).<\/p>\n\n\n\n There are no expected harms of being in this study. However, some participants may find some questions uncomfortable to complete.<\/p>\n\n\n\n WHAT ARE THE BENEFITS OF PARTICIPATING IN THIS STUDY?<\/strong><\/p>\n\n\n\n You may not benefit directly from being in this study. Information learned from this study may help physical literacy researchers and recreation programmers in the future.<\/p>\n\n\n\n WHAT OTHER CHOICES ARE THERE?<\/strong><\/p>\n\n\n\n You can choose to not participate in this study.<\/p>\n\n\n\n CAN PARTICIPATION IN THIS STUDY END EARLY?<\/strong><\/p>\n\n\n\n You don\u2019t have to be in this study. You can agree to be in the study now and change your mind later. If you choose to stop being in the study we would appreciate it if you contact us. The investigator may decide to remove you from this study without your consent if you are unable or unwilling to follow the study procedures.<\/p>\n\n\n\n WHAT ARE THE COSTS OF PARTICIPATING IN THIS STUDY?<\/strong><\/p>\n\n\n\n There are no costs associated with participating in this study.<\/p>\n\n\n\n ARE STUDY PARTICIPANTS PAID TO PARTICIPATE IN THIS STUDY?<\/strong><\/p>\n\n\n\n No, you will not be paid.<\/p>\n\n\n\n You have the right to have any information about you that is collected, used or disclosed for this study to be handled in a confidential manner.<\/p>\n\n\n\n If you decide to be in this study, the investigators and research staff will collect only the information they need for this study.<\/p>\n\n\n\n You have the right to access, review and request changes to your personal information.<\/p>\n\n\n\n The investigators, study staff and the other people listed above will keep the information they see or receive about you confidential, to the extent permitted by applicable laws. Even though the risk of identifying you from the study data is very small, it can never be completely eliminated.<\/p>\n\n\n\n The Investigators will keep any information collected about you in a secure server that is password protected for a minimum of 5 years.
